Tokyo subsidy for fertility treatment (up to ¥150,000 per treatment)

Official name
Tokyo Infertility Treatment Subsidy — 東京都不妊治療費助成事業 (formerly 東京都特定不妊治療費(先進医療)助成事業)

For treatment started on or after April 1, 2026, Tokyo subsidizes the patient's share of insured assisted reproductive treatment (IVF 体外受精 / ICSI 顕微授精) and of the advanced procedures (先進医療) done with it — up to ¥150,000 per treatment. Applications for this opened on October 1, 2026.

"One treatment" (1回の治療) means the whole course from the treatment plan to the pregnancy check after one embryo transfer — it is not split by egg-retrieval or transfer cycles. The wife must be under 43 on the first day of the treatment. The number of subsidized treatments follows the insurance rules: 6 per child if the wife is 39 or younger at the start, 3 per child if 40–42.

For treatment started by March 31, 2026 (even if it continued past April 1), only advanced procedures are covered, at 7/10 of the cost, up to ¥150,000. IVF/ICSI done entirely at your own expense, and general infertility treatment such as artificial insemination, are not covered.

Requirements are based on resident registration in Tokyo; there is no nationality requirement. Minato Ward also runs its own top-up subsidy (up to ¥300,000 per treatment for advanced procedures).

PER TREATMENT (STARTED FROM APRIL 2026)
150,000JPY
Cap on the Tokyo subsidy. Any high-cost medical care benefit (高額療養費) or extra benefit from your health insurer is deducted first. Tokyo's examples: insured share ¥90,000 + advanced ¥30,000 → ¥120,000; insured ¥100,000 with ¥30,000 high-cost benefit → ¥70,000; insured ¥120,000 + advanced ¥90,000 → ¥150,000. See also the Minato top-up (Japanese).
📢 What changed in April 2026
Treatment started by March 31, 2026: only advanced procedures, 7/10 of the cost, up to ¥150,000. Treatment started on or after April 1, 2026:
  • The insured IVF/ICSI itself is also covered (including insured male-infertility treatment in the same course)
  • No 7/10 multiplier — the costs are covered in full up to the cap, after deducting high-cost medical care benefits
  • The cap remains ¥150,000 per treatment
Applications for treatment started from April 2026 have been accepted since October 1, 2026, using a separate online form.

Who can claim this subsidy

Wife under 43 on the first day of the treatment
The age is checked on the start day of "one treatment" (generally, the day the treatment plan for one transfer was first made), not on the application day.
Number of treatments (per child)
Wife 39 or younger at the start: up to 6 per child. Wife 40–42: up to 3 per child. The count can be reset for each child. A treatment that was stopped before transfer and for which you applied also counts toward Tokyo's limit.
Married couple living in Tokyo
Legally married: married from the first day of the treatment until the application day, and either spouse registered as a Tokyo resident continuously over that period. Common-law (事実婚): able to show you are in the same household (e.g. "夫(未届)" / "妻(未届)" on the residence certificate), no other legal spouse, and both registered at the same Tokyo address over that period. Couples in separate households submit a written declaration.
Insured treatment; advanced procedures at a registered clinic
The assisted reproductive treatment must be done under health insurance. Advanced procedures count only if done at a clinic registered with the Ministry of Health, Labour and Welfare for that procedure, on or after its registration date.
Not eligible
IVF/ICSI paid entirely at your own expense (even if advanced procedures were included); general infertility treatment such as artificial insemination; treatment started when the wife was 43 or older.

Advanced procedures (先進医療) currently designated

  • SEET method — embryo culture supernatant transfer
  • Time-lapse embryo monitoring
  • Endometrial scratch
  • PICSI — sperm selection by hyaluronic acid binding
  • ERA / ERPeak — endometrial receptivity testing
  • EMMA / ALICE — endometrial microbiome testing
  • IMSI — sperm selection under high magnification
  • Two-stage embryo transfer
  • Endometrial flora test (子宮内フローラ検査)
  • Physiological sperm selection using membrane structure (microfluidic sperm sorting)
  • PGT-A — preimplantation genetic testing for aneuploidy

For treatment started on or after April 1, 2026, the insured IVF/ICSI itself is also covered.

Discuss with your fertility clinic which procedures are appropriate for your case. The latest list of procedures and registered clinics is on the Ministry of Health, Labour and Welfare website.

Application process

Deadline: March 31 of the fiscal year in which the treatment ended
"Ended" means the day of the pregnancy check after the embryo transfer, or the day the doctor stopped the treatment. Example: treatment ended August 6, 2023 → deadline March 31, 2024 (online submission date or postmark). If the treatment ended in January–March and you cannot submit by March 31, you can apply until June 30. Late applications are not accepted for any reason; if you may be late, contact Tokyo before the deadline.
How to apply
Apply online with the LoGo Form (separate forms for treatment started from April 2026 and for treatment started by March 2026); Tokyo provides an English guide for the online application. If you cannot use the online form, contact Tokyo for paper forms and send the originals by a traceable mail service.
Required documents (treatment started from April 2026)
  • Treatment certificate (不妊治療費助成事業受診等証明書), filled in by the clinic (the clinic may charge a fee)
  • Residence certificate (住民票) issued within 3 months, with relationships shown
  • Family register (戸籍全部事項証明書) issued within 3 months — foreign couples whose marriage it does not show attach a marriage certificate
  • Health insurance eligibility information for both spouses
  • Document showing the "limit certificate" category (限度額適用認定証区分) for both spouses
  • Decision notices for any high-cost medical care benefit and extra benefits received
The payment account must be in the applicant's name.
Combining with Minato top-up
Minato Ward pays up to ¥300,000 per treatment for the self-pay cost of advanced procedures, after deducting the Tokyo subsidy. Minato asks you to apply to Tokyo as well; if Tokyo's approval notice has not arrived by Minato's deadline, attach a note saying so. See the Minato page (Japanese).

Common questions

Which couples can apply?
Tokyo's page describes legally married couples and common-law (事実婚) couples. For other situations, contact the Tokyo Welfare Bureau, Maternal and Child Medical Subsidy (母子医療助成担当), 03-5320-4362.
If one treatment fails and I start another?
Each "one treatment" is counted separately. Within the limit of 6 or 3 per child, each new treatment can receive up to ¥150,000. A single treatment cannot be split into several applications, even if you changed clinics partway.
Do the new rules apply to treatment before April 2026?
No. Treatment started by March 31, 2026 uses the old rules (advanced procedures only, 7/10), even if it continued past April 1, 2026.
Does the clinic have to be in Tokyo?
Tokyo's page does not limit the clinic's location. The requirements are insured treatment and, for advanced procedures, a clinic registered with the Ministry of Health, Labour and Welfare for that procedure.
I moved out of Tokyo after treatment. Can I still apply?
Normally a spouse must be a Tokyo resident until the application day. As a special rule for FY2026, couples who started treatment on or after April 1, 2026 while registered in Tokyo, finished it by September 30, 2026, and moved out of Tokyo by October 1, 2026 can still apply by the deadline. If you are moving later, apply before you move.
