● Minato Ward / Voucher-based subsidy
● No income limit

Prenatal & infant checkups, mostly covered by 23 vouchers (up to ¥126,250)

Official name
Maternal & infant checkup vouchers (in-Tokyo) — 妊婦健康診査・産婦健康診査・1ヶ月児健康診査(都内契約医療機関)

When you submit your pregnancy notification (妊娠届) at a Minato Area Branch Office, you receive your maternal & child health handbook (母子健康手帳) together with the "Mother and Child Health Bag", which contains 23 vouchers for checkups during and after pregnancy.

Each voucher has a cap. At contracted clinics and midwife homes within Tokyo, you hand in the voucher and the Ward pays up to the cap for the items printed on it. If other tests or consultation fees push the cost above the cap, you pay the difference.

From April 2026, postpartum checkups (2) and the one-month-old infant checkup (1) are also subsidized. Their vouchers can be used for checkups on or after October 1, 2026; for checkups between April 1 and September 30, 2026, you pay first and claim the money back with an application form. If every voucher is used to its cap, the total is ¥126,250.

For checkups outside Tokyo (e.g., when returning to your hometown to give birth, 里帰り出産), there is a separate reimbursement program — see the outside-Tokyo page (Japanese).

The public payment covers the test items printed on the voucher (not covered by health insurance). Always check with the clinic beforehand that it accepts the vouchers.

Who can use these vouchers

Pregnant and postpartum women registered as Minato residents
There is no income limit. You receive the vouchers when you submit the pregnancy notification at the Health and Welfare Subsection of an Area Branch Office, after a doctor has confirmed the pregnancy. Checkups you had before submitting the notification are not subsidized.
If you move to Minato during pregnancy
Bring your maternal & child health handbook and submit your change of address at an Area Branch Office. The Ward checks how many vouchers your previous municipality issued and how many you used, and exchanges or adds vouchers if needed. If you move out of Minato, ask your new municipality.
Vouchers can only be used at contracted clinics and midwife homes in Tokyo
At a non-contracted hospital in Tokyo, no subsidy is paid. At midwife homes, the first checkup, ultrasound, cervical cancer screening and some tests cannot be done; up to 7 prenatal checkups (6 if the midwife home does not do a non-stress test) and 2 postpartum checkups can be used there. For checkups outside Tokyo, use the separate outside-Tokyo reimbursement program: pay first, then apply within 1 year of the birth.

At your checkup visit

  • Bring the vouchers and your maternal & child health handbook, and hand the right voucher to the clinic
  • The Ward pays up to the cap for the items printed on the voucher
  • If other tests or consultation fees bring the cost above the cap, you pay the difference (e.g., a 1st-visit bill of ¥13,000 against the ¥11,670 cap → you pay ¥1,330)
  • Ask the clinic about any amount you have to pay yourself
Minato Ward publishes a list of contracted clinics in Minato (also in English) on its official page; for clinics in other wards, see each ward's website. Questions: Minato Public Health Center, Health Promotion Section, Community Health Subsection (みなと保健所健康推進課地域保健係), 03-6400-0084.

Common questions

Are tests not printed on the voucher (for example NIPT) covered?
No. The vouchers cover only the test items printed on them. Anything else is paid by you.
I am expecting twins or more and need more than 14 prenatal checkups. Is that covered?
Yes, partly. For a multiple pregnancy, Minato reimburses the 15th to 19th prenatal checkups you paid for yourself: up to ¥5,460 per checkup in FY2026 (¥5,280 in FY2025), up to 5 checkups. Apply within 1 year after the birth, by post or at the Minato Public Health Center.
Can I use the cervical cancer screening voucher and the ward's free screening together?
No. Minato offers a free cervical cancer screening from July to January each year for women aged 20 and over at designated clinics in Minato. The pregnancy voucher only covers part of the cost. Check with the clinic and use one or the other.
I had a postpartum checkup or one-month-old checkup between April and September 2026. Can I get the money?
Yes. Pay at the clinic, then submit the application form (港区妊婦健康診査等費用助成金支給申請書). Checkups in that period are covered at any clinic, inside or outside Tokyo. If you submitted your pregnancy notification before April 1, 2026, the Ward mails the form about 2 months after the birth; it can also be downloaded. The postpartum checkup vouchers are only in the bags handed out from April 1, 2026. If you received your bag before then and have a postpartum checkup on or after October 1, 2026, you can also claim it with the application form.
What does the postpartum checkup include?
An interview (living situation, breastfeeding, worries about childcare, history of mental illness, medication), an examination (recovery of the uterus, lochia, breasts), weight and blood pressure, and a urine test. Use it within 2 months after giving birth.
